Welcome to Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is a dedicated multidisciplinary team providing intensive, community-based services for adults with Severe Persistent Mental Illness (SPMI), including conditions such as Schizophrenia, Schizoaffective Disorder, and Bipolar Disorder. This specialized wrap around community based service is designed for adults, 18 and older.

Our Services: Our comprehensive range of services is designed to support individuals facing significant challenges, and includes:

  • Housing Supports: Assistance with securing and maintaining stable housing.
  • Medication Support: Regular monitoring and assistance with medications.
  • Independent Living Skills: Training and support for daily living activities.
  • Therapy: Access to therapeutic services tailored to individual needs.
  • Vocational Skills Training: Helping individuals develop skills for employment.
  • 24/7 Crisis Response: Immediate support during crises.

Who We Serve: This service specializes with individuals experiencing challenges such as:

Difficulty attending mental health appointments in the community
Difficulty obtaining and maintaining medication schedules
High use of crisis services
Multiple psychiatric hospitalizations
Homelessness or risk of becoming homeless
Substance use challenges
Difficulty keeping stable employment

Our Approach: ACT is designed to meet individuals where they are, providing personalized support that helps manage and improve their mental health and overall quality of life. Our goal is to reduce hospitalizations, prevent crises, and help clients achieve greater stability and independence.

No Fee for Service: Our services are funded by your local Managed Care Organizations, so there is no charge to guests.
